Patient Policies

Cancellations

We ask that  you please call the office 24 hours in advance to cancel appointments. Patients that skip or miss appointments may be subjected to a $50.00 charge, for which you are responsible, since insurance will not cover this fee.  We ask that all patients please keep in mind that missing appointments or cancelling appointments at the last minute leaves other patients unable to be seen in a timely manner.

Late Policy

We are committed to prompt service, and will work very hard, barring emergencies, to stay on time. We may ask you to reschedule if you arrive for your appointment more than 15 minutes after your scheduled appointment. Always arrive 10 to 15 minutes early for an appointment to complete any paperwork associated with your visit.

Referrals

You should call the office at least three (3) days before the appointment to give us the information needed to complete the referral. That information includes: your name, the physician you are being referred to, his phone and fax numbers, the date of the appointment, the diagnosis for the referral, your insurance, whether it is for a visit or a specific procedure (such as surgery), and where it is going to be performed (office or hospital). We do not mail referrals to specialists. You should familiarize yourself with your insurance plan, and determine if it requires you to take a referral with you; if you are not certain of how your referral is handled, please ask our office staff, and they will help you to determine what to do.

Lab and X-Ray Results

For lab and test results, please call and speak with the nurses at FPA of West Grove. The nurses may give information over the phone, but not before the doctor has reviewed the test results. In many circumstances, the staff may ask you to schedule an office visit to discuss test results with the doctor.

Prescriptions

Please call and leave a message on the Nurse's phone line for prescription medicine refills.  Please try to call the office at least 48 hours in advance for all medications.  Always leave your full name, date of birth, name of the medicine(s), spelling it out if necessary, the prescription number on the bottle, if the prescription is for one month, three months, or other time period, your phone number and the name of the Pharmacy (include the address if a chain store like Walgreen's or CVS).

Emergencies and On Call

If you have an emergency, such as chest pain, severe shortness of breath, severe headache, or bleeding, call 911 or proceed directly to the nearest hospital emergency room. You can always call the office and ask to speak with a nurse in regards to  acute medical problems.  Please be aware that the nurses and staff may be unable to answer your calls immediatedly and if your condition worsens, you should seek attention in the emergency room.

If there is a medical emergency after hours and you need to speak to the on call physician, please call our regular office number (610-869-4627) and you will be directed on how to reach the on call physician directly.

Records Releases

To obtain your medical records from your previous family physician and any specialists you may be consulting, this form can be sent directly to the physician's office.  Records Release Form
